Is there a time limit that the insurance company has to approve or deny a claim? (I've heard that after 45 days, if they haven't made a decision, then they have to pay by default.) Is that true?

Each insurance company's claim department is regulated by each state's Department of Insurance. The time allowed to accept or deny a claim would fall under that state's jurisdiction. You can follow this link to your state's Department of Insurance.
