If the car is insured under my name, and a friend was driving it and got into an accident, what would happen?

In most cases insurance follows the car first and driver second. This means that you, as the owner, will be liable for the damage and have to use your insurance policy primarily. If the damages exceed your coverage limits the driver's insurance would be used as secondary or excess coverage.
