In most states it is allowable for the insurance company to make the check payable to the insured and any lien holder or even make the repair facility the payee. So it could be any of the three entities you listed or a combination of a couple of them.

If the check for your hail damage is made out to both you and the lien holder you will likely have to meet up with them to have them endorse it so you can use the money for the repairs.
