When you obtain at least the minimum car insurance coverage that your state is requiring of you to obtain and maintain then the insurance company will fill out the SR-22 form and file it with the state on your behalf. Not all insurance companies file the SR22, certificate of financial responsibility, so you will need to make sure the insurance provider you get your auto insurance policy through does offer the filing of the SR22.
An agent can provide you the actual form, or it will be mailed to you with your policy documents. Each state has different form requirements, but typically it is a pink form that has your name, policy number, etc. It is used in the drivers license office to show proof of your compliance. The carrier will also electronically or manually (depending on the state) file with the Motor Vehicle department an acknowledgement that you have the SR-22.
