When the PIP law went into affect 01-01-08, health care providers are require to submit a health provider certificate, the insurance adjusters are sending back our claims asking for this information. My question is where do we get this form from? Could you please let me know the form # and how I can obtain it for the adjusters. Thank You

If appears you are referring to the No-Fault reforms that took place in Florida in 2007 and which went into effect as of January 1, 2008.

The Florida Office of Insurance Regulation (OIR) has created the form titled Health Care Provider Certification of Eligibility for PIP Benefits and has it available on their website for download. So if you go to this OIR link you can print out the form you need to provide to insurance adjusters as a health care provider tending to injuries from an auto accident.

The Completion of the form is necessary for a health care provider to become certified to perform reimbursable medical procedures under the Personal Injury Protection (PIP) insurance law reforms passed by the Florida Legislature in October, 2007.

The new FL law (Florida Statute 627.736) requires medical providers to meet specific certification criteria in order to provide treatment to patients covered by PIP. In order to be eligible for reimbursement, providers will be required to affirm, under penalty of perjury, that they are statutorily authorized to treat PIP patients.
