So if you go to this OIR link you can print out the form you need to provide to insurance adjusters as a health care provider tending to injuries from an auto accident.

The Completion of the form is necessary for a health care provider to become certified to perform reimbursable medical procedures under the Personal Injury Protection (PIP) insurance law reforms passed by the Florida Legislature in October, 2007.

The new FL law (Florida Statute 627.736) requires medical providers to meet specific certification criteria in order to provide treatment to patients covered by PIP. In order to be eligible for reimbursement, providers will be required to affirm, under penalty of perjury, that they are statutorily authorized to treat PIP patients.
