The National Highway Transportation Safety Administration (NHTSA) states that the NDR) contains only a listing of names and related identification, provided by State driver licensing officials, of those "problem drivers" whose driver's licenses have been cancelled, denied, revoked, or suspended or who have been convicted of certain serious traffic violations.
To have the NDR checked to see if you are listed on it, complete the form, have your signature (or your mark as witnessed) notarized, and mail the completed form to the National Driver Register at: 400 7th Street, SW., Room 6124A (NPO-124), Washington, DC 20590-0001. Forms that are not notarized will not be processed.
All mailed NDR record check requests must be notarized to certify the identity of the requester. To get the form notarized you will have to find a notary public also just known as a “notary.” These are individuals who the state has been commissioned by state authorities to administer oaths and affirmations, witness signatures, and perform other duties as permitted by state law. Often, employees of large institutions such as banks, insurance companies, real estate, and law offices are notaries public. Also, many people who work at courthouses are notaries. Notaries are normally permitted by the state to charge a nominal fee to notarize a document.

So to get your form notarized you will need to find a notary in your area. You may try a bank, the courthouse or even find one in the yellow pages. You will need to bring the record request form and proof of your identity, usually a driver’s license will do. The notary will then certify that you are the person listed on the form fill out the notarization section and stamp the document with their official notary stamp. The stamp usually listed their name, state county and notary number given to them by state as well as the expiration date of their commission.

Once your form is properly filled out and sent in the NDR states it will respond to every valid inquiry including requests that produce no record(s) on the NDR file.
When records are located, details of the probable identification results will be returned to you and will contain all information listed in the NDR records, if any, on you. The reply will also indicate any disclosures (reports to others) previously made by the NDR and will specify whom, if anyone has received reports on you.
