A SR-50 is a form used by a person to provide proof of current insurance to the Indiana Bureau of Motor Vehicles (BMV). This form shows the Bureau both the beginning and end dates of the current policy.

The Indiana government site has the Affidavit of Current Insurance "SR-50" form online if you wish to look it over, print it out and get it filled out before going into the BMV to save time at the Motor Vehicles office. However you will not only need to fill out your personal information that it requests on it but will have to have your insurance agent fill out the insurance policy information that is requires and sign it that this information is correct.

There is a specific place on the SR50 form for the signature of an authorized representative from your insurance company. The IN BMV notes that a fraudulent signature on the form will result in the suspension of your driving privileges.
